Evergood Social Care are looking for an experienced and qualified social worker to work in an Adult’s Hospital Team based in Essex.
The HAT team is a social work service based within the hospital, working with patients and their families and carers, who are admitted to hospital. Our tasks are to assess and plan and support service users (SU’s) to cope with social, emotional, economic and environmental problems. We also help SU’s to maintain their independence and to live as normal a life as possible in the community and to work jointly with health staff on social matters that may affect present and future health. We will often draw upon domiciliary, day and residential services to support SU’s on discharge from hospital.
We aim to help service users to remain as independent for as long as possible. Ensure that SU’s and their families/carers are involved at every stage in the planning and delivery of the service we arrange for you; provide support services which are available to all, whatever the gender, age, race or religion, cultural beliefs, sexual orientation or disability; take a multi-disciplinary approach.
We will work with other professionals within the hospital setting adopting a multi-disciplinary approach. Other professionals will include physiotherapists, OTs (occupational therapists), nurses, mental health liaison nurses, speech and language therapists, and doctors/consultants. In many cases there will be a multi-disciplinary meeting which will make recommendations on how to meet the SU needs after discharge from hospital. This may include continuing Health Care recommendations for funding post discharge.

To be considered for this opportunity, you will have:
- 3 Years Post Qualified Experience
- Previous Adult Hosp experience
- Social Work Qualification (Dip, BA, MA etc.)
- Hold a full UK driving license
- Have a current HCPC registration
