Senior Practitioner – Older Persons

To ensure that the practice of team meets statutory requirements and provides a safe, appropriate and expedient response to customers.

To lead and support team members to understand and meet the statutory performance indicators and to report on appropriate local performance targets to the Team manager.

Ensure that the Care management Team are fully trained, competent and confident in the effective delivery of the Safeguarding agenda and that it is integral to the teams’ day-to-day practice.

To monitor effective data recording of all case records in line with best practice and standard of record keeping

To ensure practice delivers safeguarding agenda

To provide professional leadership by providing case management guidance, support and expert advice to the team, students and partners

To ensure that professional expertise is maintained and constantly updated in line with the necessary regulatory body Where relevant lead appropriate NHS staff within agreed protocols

To safeguard those identified as vulnerable in line with core roles and responsibilities, and to provide management guidance, advice and oversight to team members

To record, investigate and respond to safeguards, risk management, complaints and provide detailed and professional responses in accordance with related standards and timeframes.

To provide professional and management leadership whilst still holding and managing a complex caseload, by providing case management guidance, support and expert advice to the team, students and partners.

To ensure that personal professional expertise is maintained and constantly updated in line with the necessary regulatory body.

To identify appropriate systems of support with customers and their carers to enable them to maintain / improve their quality of life and function as independently as possible.

To be aware of the Council’s aims, organisation values and behaviours and their impact on this post and the service delivery of the team. Understand, implement and participate in the Council’s performance management process as required.

Where relevant, lead appropriate NHS staff within agreed protocols for clinical staff deployment, allocation of work and staff development and adhere to local arrangements for clinical guidance.

Ensure that staff understand and implement the Council’s policies and procedures.

To carry out the duties of the post in accordance with the Council’s equality and diversity policy.

The job description will be subject to annual objectives which will be developed in conjunction with the post holder. It will be subject to regular review and the Council reserves the right to amend or add to the duties listed.
